The Health Information Management (HIM) department is dedicated to safeguarding the integrity, privacy, and confidentiality of your medical records. We handle all health information requests with care, in full compliance with HIPAA and other applicable laws.
Whether you’re requesting records for yourself, a provider, or a legal entity, our team is here to assist you through the release of information process efficiently. To ensure timely processing, all requests are required to use our official Release of Information (ROI) Authorization Form.
All records requests should be faxed to 480-566-1165 with a signed ROI authorization form. Patients requesting records may also come in to our HIM office at RPHC to request records. The HIM office is located on the 1st floor main front desk area.
